What you'll need
Every family completes the enrollment application. These documents are required before your student's first day.
- ✓Maryland Emergency Form (OCC 1214) — completed inside the application
- ✓Maryland Record of Physical Examination
- ✓A copy of your Maryland Homeschool Notification Form submitted to Prince George's County Public Schools
- ✓Signed payment agreement
- ✓Birth certificate and immunization records (or applicable exemptions)
- ✓IEP, 504, or evaluation reports if your student has them
